Managing Prescription Costs with the Medicare Prescription Payment Plan

When you’re coping with cancer, a large prescription bill can add to an already stressful time. If you have Medicare drug coverage, the Medicare Prescription Payment Plan offers a way to spread your out-of-pocket costs for covered prescriptions across the calendar year.

Understanding how this option works can help you decide whether it fits your budget. 

How the payment plan works

The Medicare Prescription Payment Plan is a free, optional program available to anyone with Medicare prescription drug coverage, including Medicare Advantage plans with drug coverage.

Once you’re enrolled, you receive a monthly bill from your health or drug plan instead of paying at the pharmacy for your covered Part D prescriptions. You still pay your plan premium separately, if you have one.

The payment plan changes when you pay. It does not reduce the total cost of your prescriptions. Learn more about the Medicare Prescription Payment Plan.

What to know about the 2026 drug cost limit

In 2026, your annual out-of-pocket costs for prescriptions covered by Medicare Part D are capped at $2,100. This limit applies automatically to everyone with Medicare drug coverage, whether or not they join the payment plan. It applies to covered Part D drug costs, not all of your health care expenses.

The payment plan may be useful if you have high prescription costs, especially early in the year. Signing up earlier gives you more months to spread out those costs.

Your monthly bill can change. As you fill prescriptions, new costs are added and fewer months remain in the year to pay them. If your drug costs are low or you join late in the year, this option may be less helpful. Ask your plan what your medication payments could look like. How to get started

You can enroll at any time during the year through your Medicare health or drug plan. Call your plan or visit its website to ask about joining. Enrollment takes place through your plan, not your pharmacy and all of your covered Part D prescriptions are included.

You can also leave the payment plan at any time. You’ll still need to pay any remaining balance, and you’ll pay the pharmacy directly for new covered prescription costs after leaving. Medicare has more information about participating in or leaving the payment plan.
